If you prefer, you may send e-mail to firstname.lastname@example.org or email@example.com. You may also choose to send a request through Connexion or Record Manager. In Connexion select Report Error message under the Action menu. In Record Manager select Send Record Change Request under the Record button.
If the change or addition you are requesting requires proof (hardcopy), then please PRINT THE SUMMARY of this form (you will get an emailed copy) and either fax or mail it to OCLC.
