Questionnaires for preservation services

The Request for Proposal (RFP) questionnaires on this page have been designed to help you supply the information we need to write a complete and accurate proposal for your project.

Please feel free to contact us if you have any questions as you complete the form that is appropriate to your type of project. Once we receive your completed questionnaire, OCLC Preservation Service Centers will prepare a proposal and price estimate for your project. We will contact you if we need any additional information or clarification to prepare your proposal. Estimated proposal completion dates can be provided upon request.
