Employee Assistance Program (EAP)Confidential counseling and referral services provided for employees and their families. Up to eight sessions provided at no fee. Counseling for a variety of personal difficulties such as, but not limited to, relationships, marriage, family, stress, grief, substance abuse, other addictive disorders, and child and adolescent issues. Counseling may take place in individual or group sessions, depending upon preference. It is a taxable benefit. Employment Insurance (EI)Government provided program: Up to 55% of average insured earnings up to the statuary cap. May be used after Sick Pay is exhausted and prior to the start of Long Term Disability Long Term Disability (LTD)The Long Term Disbility Benefit is 60% of monthly earnings to maximum $10,000 per month or 85% of pre-disability take-home pay, whichever is less to age 65. There is a 90 days waiting period. Proof of insurability needed for benefit over $3,700 per month. The definition of total disability: for first 24 months, unable to perform your own job; thereafter, prevented from performing any job. The LTD benefit may be reduced for certain other income. OCLC pays the premium for this benefit. This is a taxable benefit. Life Insurance BenefitsEmployee Basic Life Insurance:Employee Basic Life Insurance: 300% of annual earnings, with a maximum of $500,000. This benefit is reduced by 50% at age 65 and terminates at age 71. Proof of insurability required for any coverage above $158,000. Benefit terminates upon termination of employment. Waiver of Premium after 90 days of disability. Conversion privilege under age 65. OCLC pays for this benefit. It is a taxable benefit. Dependent Basic Life InsuranceThis benefit covers an employees spouse for $10,000 and dependent child for $5,000. Waiver of Premium for disability if premium waives for Basic Life. OCLC pays for this benefit. This is a taxable benefit. Optional Life InsuranceEmployees and/or spouses can purchase optional life insurance in units of $10,000 to a maximum of $200,000. Proof of insurability required for any coverage. Waiver of Premium for disability if premium waives for Basic Life. Coverage stops at age 65. This insurance can be converted to individual policy. This benefit is paid for by the employee. This is a non-taxable benefit. Employee Accidental Death, Dismemberment, and Specific Loss (AD&D)InsuranceEqual to the Employee Basic Group Life Insurance. Death or loss within 365 days of date of accident. Schedule of losses includes life, hand, foot, eye or combination. This benefit terminates at age 71. This benefit is payed for by OCLC and is taxable in Quebec. Business Travel Accident24 hour coverage while traveling on company business. Covers all employees and includes exposure and disappearance provisions. Covers death, dismemberment or loss of sight. OCLC pays for this benefit. Group Registered Retirement Savings Plan (RRSP)OCLC AccountOCLC makes contributions of 5.5% of base salary on a monthly basis, and no employee contributions required. Employees are eligible after 1000 hours of work and 12 months service. Employees are vested immediately in all Employer contributions. Withdrawals are permitted only for financial hardship, or medical expenses, or purchase of primary residence or for higher education. This plan is administered by Standard Life Assurance Company. Employee AccountEmployees may make contributions to an Employee Account. They may contribute from 1-6% of earnings and OCLC will match 35 cents for every dollar contributed. Employees are eligible after 3 months of service. The administrator for this plan is Standard Life Assurance Company. Employee Voluntary AccountEmployees may make additional voluntary contributions over and above the 6% up to maximum allowable limit set under Income Tax Act (currently - 18% of earnings up to $16,5000 per year - as of 2005). Eligibility is reached after 3 months of service. Withdrawals are permitted at any time from Employee Voluntary Account. This account is administered by Standard Life Assurance Company. Spousal AccountContributions may also be made of behalf of spouse (RRSP Spousal). These contributions can not be from matched funds, but must be from contributions over 6% of base salary. The spouse (not employee) is permitted to make withdrawals. The administrator is Standard Life Assurance Company. LeavesVacationVacation leave is an earned benefit that accrues on the basis of each full month of employment. Vacation is not earned until the respective month is completed

Reimbursement (rate is set annually) is $C 500 per employee per calendar year in 2005. Note: This does not include the costs incurred for fitness classes. This applies to fitness facility memberships only.
